Witness Account

What the witness personally observed — in their own words.

Rep guidance — capturing the account Ask the witness to tell you what they saw and heard in their own words. Record as close to verbatim as possible. Then ask follow-up questions:
  • "What did you see happen?"
  • "What exact words did you hear?"
  • "Were there others present who saw this?"
  • "Has this happened before that you know of?"
Keep this section to what the witness directly observed. Their opinions and your assessment go in separate sections below.

Anything the witness wants to add in their own voice.
