Do not include: your opinions about what happened, prior history you know about this person, or anything you didn't personally witness. Those details can complicate your position — leave them out.
Stick to observable facts — what you saw, what you heard. Avoid conclusions or characterizations.
Who did you notify, what did you say or do?
If yes, it will be attributed as hearsay — e.g., "I was informed by…" Leave blank if not applicable.
